- 2024/2025 KESSA Receipt (₦1,000 payment proof)
- Confirmation of Admission (FUBK admission letter)
- Student Identity Card (Valid FUBK ID)
- Local Government Indigene Certificate
- Primary School Certificate
- Payment Remita/Invoice (Scholarship fee payment proof)
- Form 02 (Academic status verification form)
- Extension Letter from HOD (For spillover students)
- One Recent Passport Photograph
